Where did you get this version of gfortran?  From the links in the gcc
wiki?  You seem to assume we know what you're talking about but there
could be any number of random people on the internet that have offered
compiled versions of gfortran for you to download; we have no idea.  If
you are using the ones from the Wiki then you should follow the
installation directions there.  We don't really have anything to do with
those binaries so you should ask on their mailing list.

We also offer gfortran as part of our gcc 4.2 packages.  If you want to
use our gfortran you should download those, they are the only ones we
support.  They simply unpack in the root where you've installed MinGW. 
You'll want the gcc-core and gcc-gfortran packages.  They are in two
flavors, sjlj and dw2.  If you're not sure what this means, then choose
the sjlj flavor.  These are all downloadable at the MinGW sourceforge
download site.

Don't know, ask them.

If you are using our gcc 4.2 packages, they must be extracted to the
root where you installed MinGW.

Do note that none of these will contain g95.  g95 is a separate project
that is not part of gcc.  It was an old fork.  gcc < 4.0 comes with g77,
gcc >= 4.0 dropped g77 and uses the newer rewritten gfortran.
